Aetna: Basic HMO, HMO Open Access and POS plans available in the 2+market and may be carved out to one life. Available to firms in the tri-state area as well as nationally. The HMO Open Access does not require a referral to see a specialist. POS plans are based on an 80th UCR for out-of-network benefits.

AmeriHealth: Insuring NJ based companies since 1995 with over 600,000 members. Amerihealth is the wholly owned subsidiary of Keystone BC/BS of PA offering a wide range of plan options, HMO's, POS, and PPO's utilizing the Multi-plan national network of doctors in the 2+ market.

Atlantis Health Plan: New to the marketplace, Atlantis offers both HMO and POS plans to New York City based businesses. The HMO plans utilize their existing network while the POS plans utilize the Multiplan network.

CIGNA: POS plans and HMO plans in New York and New Jersey. The network is a combination of the former Healthsource network and the CIGNA network throughout the tri-state area. CIGNA's HMO plans are among the most competitive in the area and they recently streamlined pre certification and referral policies.

GHI: The GHI PPO plans, utilize the full GHI PPO network. These plans are available to firms throughout New York State from Buffalo to Montauk. Co-pays are available in either $5, $10, $15 or $20 with deductibles of $500, $750 or $1,000. Out of network reimbursements are based on HIAA at a choice of 70%, 80% or 90%. The GHI PPO network is one of largest and includes providers not only in NY, NJ and CT but also Florida, Arizona, Nevada, Puerto Rico and other areas. The PPO plans require 50% participation. Soon this network will be available as an EPO with much lower rates. The new EPO plan will be offered with HMO enrollment procedures, meaning no participation requirements.

Guardian / Healthnet: HMO and POS Charter plans that allow the insured direct access to the full network with NO referral from a gatekeeper. Available in New York, New Jersey and Connecticut. HealthPass: This New York alliance offers small group plans from four companies on one application. Participating companies are GHI, Horizon, HIP and Healthnet. Currently available New York City, Nassau, Suffolk, Westchester and Rockland based businesses employing between 2 and 50 people. This program enables the small employer to offer their employees a choice of networks and programs that is otherwise unavailable in this market.

HIP: HIP is one of the pioneers in health care and one of the largest Not For Profit health care companies on the East Coast. HIP plans are available to New York companies in either HMO or POS plans.

Horizon Healthcare: Horizon Healthcare brings 65 years of experience to New York with PPO, EPO and HMO plans available with deductibles of $250 to $3,000 and co-pays of $10 or $20 or $30. Available to firms in New York, the PPO plans utilize either the national Multi Plan network or Horizon's own VISTA network with more competitive pricing. Horizon also offers a Vista EPO plan which utilizes in network deductibles and has no participation requirements making this one of the unique ways to offer a very inexpensive health plan.

Oxford Health Plans: POS plans as well as Non gatekeeper POS Open Access plans (Oxford Select) with two different networks, Freedom and Liberty. The Freedom network is recognized as one of the most outstanding in the tri-state area for both PCP's and specialists. Both networks cover the tri-state area and all plans are available in New York, New Jersey and Connecticut. For employers with out of area employees the Oxford USA program is now available nationwide. In addition Oxford offers their new Metro Plans in both the Freedom and Liberty networks with dual co-pays and some higher in network co-pays to lower the cost.

United Healthcare: This highly regarded national company specializes in offering non gatekeeper POS plans (Choice+) . Dual option available for plans of 10 or more lives. Their excellent networks' cover the tri-state area. Unique telephonic referral system for POS and HMO plans allowing the insured access to a specialist for up to one year per diagnosis.
