HMO plans utilize their existing network
while the POS plans utilize the Multiplan

York and New Jersey. The network is
a combination of the former Healthsource
network and the CIGNA network throughout
the tri-state area. CIGNA's HMO plans
are among the most competitive in the
area and they recently streamlined pre

GHI: The GHI PPO plans, utilize the
full GHI PPO network. These plans are
available to firms throughout New York
State from Buffalo to Montauk. Co-pays
are available in either $5, $10, $15
or $20 with deductibles of $500, $750
or $1,000. Out of network reimbursements
are based on HIAA at a choice of 70%,
80% or 90%. The GHI PPO network is one
of largest and includes providers not
only in NY, NJ and CT but also Florida,
Arizona, Nevada, Puerto Rico and other
areas. The PPO plans require 50% participation.
Soon this network will be available
as an EPO with much lower rates. The
new EPO plan will be offered with HMO
enrollment procedures, meaning no participation
requirements. |

on one application. Participating companies
are GHI, Horizon, HIP and Healthnet.
Currently available New York City, Nassau,
Suffolk, Westchester and Rockland based
businesses employing between 2 and 50
people. This program enables the small
employer to offer their employees a
choice of networks and programs that
is otherwise unavailable in this market. |

York with PPO, EPO and HMO plans available

network or Horizon's own VISTA network
with more competitive pricing. Horizon
also offers a Vista EPO plan which utilizes
in network deductibles and has no participation
requirements making this one of the
unique ways to offer a very inexpensive

Freedom and Liberty. The Freedom network

available in New York, New Jersey and
Connecticut. For employers with out
of area employees the Oxford USA program
is now available nationwide. In addition
Oxford offers their new Metro Plans
in both the Freedom and Liberty networks
with dual co-pays and some higher in
network co-pays to lower the cost. |
