| Aetna: |
Aetna
is currently the best carrier for groups with
multi sites. They are one of the largest insurance
providers in the country, with strong networks
available through out the United States. Aetna
offers HMO, POS gated or open access and PPO.
Aetna also provides ancillary coverage including
life, AD&D, LTD and Dental.

| Amerihealth: |
Amerihealth
is wholly owned subsidiary of Keystone BC/BS.
Amerihealth currently writes NJ based businesses
exclusively with strong networks throughout
the entire state of New Jersey.

| CIGNA
Healthcare: |
Cigna
Healthcare offers a wide variety of products,
including HMO, POS, non-gatekeeper POS and
PPO products. Cigna is a national carrier
with networks set up across the country. CIGNA
also offers domestic partnership coverage.

| Empire
Healthcare: |
Empire
Healthcare's primary focus consists of the
New York market place. Empire uses the Blue
Cross/Blue Shield network locally offering
HMO and PPO products. The PPO product allows
member to use the Blue Cross/Blue Shield national
network via the "Blue Card".

| GHI: |
Group
Health Incorporated (GHI) is primarily focused
in the New York area, with networks throughout
New York State. GHI offers PPO and EPO products,
and their PPO product has multi-sited capability
using a combination of the GHI network and
Multiplan.

| Healthnet:
(Formally PHS) |
Healthnet
offers PPO, POS and NGP plans. Their strongest
networks are currently in New York, New Jersey,
Connecticut, and Eastern Pennsylvania. Healthnet
has multi-sited capability on a limited bases
or in conjunction with an arrangement made
by Healthnet with Guardian. . Healthnet is
recognized for their highly competitive networks
and excellent customer service.

| Horizon
Healthcare: |
Horizon
offers HMO, EPO, and PPO products. In New
York and New Jersey they use their own networks
while utilizing the Multiplan network for
the balance of the country, for multi-sited
cases. Their EPO product can be written with
as few as 5 enrollees for up to 150 lives.
Horizon recently purchased Anthem network
in the tri-state area.

England: |
New
England is a national carrier, which focuses
primarily in the self-funded and partial self-funded
market place.

| Oxford
Healthplans: |
Oxford
Healthplans offers one of the strongest regional
networks with primarily POS plans with or
with out gatekeepers. Oxford utilizes the
First Health network for multisided clients.
Oxford also offers PPO cost sharing plans
designed to compete in the low option market,
especially with HMO's.

Unicare
exclusively focuses on the Large Group Market.
Unicare is a national carrier that offers
a competitive PPO product. Unicare unitizes
PPO networks through out the United States,
with strong regional PPO networks in place.
Please note Unicare requires a minimum of
51 enrolled.

Healthcare: |
| United
Healthcare is a national carrier, with networks
in place throughout the United States. United
Healthcare has a strong network in the Tri-State
area. United Healthcare offers, HMO, POS,
open access POS and PPO plans. United is nationally
accredited for their outstanding customer
service. |
