What is HealthPass?

A small group plan only available to New York City employers, offering a choice of 20 different health options from four leading insurers; GHI, HIP, Horizon, and PHS.

Is there an additional cost to the Employer?

No, and there are no membership fees.

How many plans can be offered to the employees?

Twenty different plans; which consists of "in-network only", "in and out-of-network" and other options.

Are there multiple enrollment forms?

No, one enrollment form, and one bill with one monthly check.

What are the eligibility requirements for HealthPass?

Health Pass is offered to small business with 2 to 50 full-time, eligible employees. The business street address must be located in one of the five boroughs of New York City.

Is there an employee eligibility requirement?

Yes, 75% of your eligible employees must enroll in Health Pass or have spousal or governmental coverage.

Are there providers only in New York City?
No, HealthPass providers are located throughout the tri-state area.
